영어(미국) 관련 질문

I came across the below sentence while studying with Callan Method.

“Do you always answer emails the instant you receive them?”

But I don’t quite understand how this sentence is structured grammatically.

In specific, I can’t understand why it’s possible that the two nouns (emails, the instant) can be in a row.

Could anyone explain how this sentence is constructed?

Yours sincerely.
